• ANY patient who has ascites, limb amputation, cystic fibrosis, endocarditis, major burns, Cisplatin Chemotherapy, renal transplant.

1. Request/Care Plan
2. Search: Gentamicin
3. Select: MEDS: Gentamicin once daily loading dose (adult)

4. Scroll down & Click – Form symbol CHFT Dose Calculator

5. Form will appear – Input: Age, Height, Weight, Sex, Creatinine. And it will give you the dose.

6. Return to EPR and select the correct dose from the list (and continue order process as usual)
